WE Connect Resource Fair

Description

SUNDAY, residents of San Francisco and neighboring communities are invited to take part in a resource fair hosted by WE Connect in partnership with United Way Bay Area, Intuit, Tax-Aid and San Francisco Health Plan. The event will allow community partners the opportunity to share numerous money saving and health care resources critical to local California families. Some of the resources available include free health care application assistance, free tax preparation, free flu shots and dental screenings, and a healthy food giveaway.

Please bring these documents to apply for a health coverage program:

• Proof of identity (California ID, driver’s license, passport, etc) for each family member applying)
• Proof of California residency (California ID, driver’s license, rental/lease agreement, or utility bill)
• Proof of income Tax Return: 1040/1040A/1040EZ, paystubs, award letters within the last 45 days, (unemployment, disability benefits, social security, signed letter from the employer)
• Proof of childcare or child support expenses
• Proof of citizenship or immigration status – not required for some programs (Birth certificate, passport, permanent resident card, etc)
• Proof of Assets (required for adults applying -bank statements within the last 45 days, most recent stocks/mutual fund statements, including those for retirement accounts)

Please bring these documents to file taxes:

• Copy of 2010 tax return, if available
• Social Security Number or Individual Taxpayer Identification Number (ITIN) for each family member
• Proof of Income including 2011 W2s and 1099 forms
• Documentation of deductible expenses
• Account and routing numbers of checking and saving accounts for direct deposit and a faster refund
