Onlok Lifeways is a comprehensive health plan serving seniors who live in San Francisco, Fremont, Newark, Union City or Santa Clara County (not including Gilroy, Morgan Hill or San Martin). This program provides an important alternative when nursing home care and placement seem necessary. With On Lok Lifeways, a senior can remain within the community, enjoying the comforts of home and family, for as long as possible inclusive of medical and personal care assistance. With the support of a comprehensive package of services, care is coordinated and delivered by a dedicated team of:
} Physicians
} Nurses
} Social Workers
} Physical & Occupational Therapists
} Dieticians
} Transportation Staff
} Home Care Workers
} Recreational Activities Leaders

Each participant's needs are monitored and addressed. Based on participant and caregiver input, the team puts together an individualized healthcare plan and then continuously assesses the participant's status. Treatment and services are adjusted to suit the member's situation as healthcare needs change.

I have been with On Lok Lifeways for 4 yrs. I started in our San Francisco and Fremont Centers. As an Enrollment and Outreach Specialist my responsibilities are to educate the general public and agencies about what services On Lok Lifeways offers seniors. My background is in International Economics, however I have always gravitated towards the social services field. I feel that I make more of a difference in this field then in economics. I have worked at medical and governmental agencies for over 27 yrs. I have worked with abused women and children, at risk youth and ex-convicts with the County of Stanislaus. Helping others has always been a passion of mine. I have an extensive experience in serving a multicultural population. I speak Spanish and some conversational Portuguese.
