Heart & Soul Care Management offers comprehensive medical & psychiatric care management for the elderly and chronically ill adults, who need assistance with disease management and the activities of daily living.  We provide oversight to assist with maintaining stability of care.

We provide personalized and specialized care management and
consulting to clients and families.  We also work with health care
professionals including Physicians, Hospital Case Managers/Discharge Planners, Insurance Providers, social workers and related practitioners. 

Heart & Soul Care Management & Consulting was developed from my
background in medical social work, hospital discharge planning, 
psychiatric emergency, acute and transitional rehabilitation and elder
home health care management.  In these situations, I saw a critical need for support for continuity of care outside of the acute care facilities. 

We are a liaison for busy families and for families at a distance, whose loved-one may need oversight of their care, in order to remain safe at home.
